Incident investigation concluded
The Norwegian Petroleum Safety Authority (PSA) has concluded and published
their investigation of the fatal incident on Maersk Interceptor 7 December 2017.
The incident investigation report concludes that a raw water pump fell to sea as a consequence of the failure of a flat-braided sling used in a lifting operation. The lifting operation was part of the installation of a raw water pump. The incident resulted in one fatality and one person seriously injured.
“Both Maersk Drilling and our customer Aker BP support the findings of the PSA report, which are consistent with those of our internal investigation. The incident on Maersk Interceptor shocked us all and should not have happened. We have given full cooperation to the PSA investigation and are committed to do everything in our power to prevent a similar tragedy from happening again,” says Angela Durkin, COO of Maersk Drilling.

Maersk Drilling has fully complied with the Notice of Order (NoO) from the PSA received in December 2017. The NoO was aligned with the findings of Maersk Drilling’s internal investigation and was related to lifting equipment and installation of raw water pumps. An Order Response Group was established and improvement opportunities within technical, operational and organisational barrier elements were identified and are being implemented.
In addition, a Learning Verification Group has been established to oversee Maersk Drilling’s work to effectively implement the actions from the Order Response Group as well as the recommendations from the internal investigation report and any corrective measures required to address the deviations identified in the PSA investigation report.
